Patient's Responsibilities

•The care a patient receives depends partially on the patient himself.  Therefore, in addition to these rights, a patient and family member has certain responsibilities as well.  These responsibilities should be presented to the patient in the spirit of mutual trust and respect.

•The patient has the responsibility to provide accurate and complete information concerning his/her present complaints, past illnesses, hospitalizations, medications, and other matters relating to his/her health.

•The patient and family member are responsible for reporting perceived risks in their care and unexpected changes in their condition to their responsible practitioner. 

•The patient and family are responsible for asking questions when they do not understand what they have been told about the patient’s care or what they are expected to do.

•The patient, family are responsible for following the treatment plan established by his/her physician, including the instructions of nurses and other health professionals as they carry out the physician’s orders.

•The patient, family member is responsible for his/her actions should he/she refuses treatment or not follows his/her physician’s orders.

•Be fully informed in advance about the care/service to be provided including the disciplines that furnish care/service and the frequency of visits as well as any modifications to the plan of care.

•Be informed, both orally and in writing, in advance of care/service being provided, of the charges, including payment for care/service expected from third parties, and any charges for which the patient will be responsible.

•Received information about the scope of services that the facility will provide and specify limitations on those services.

•Participate in the development and periodic revision of the plan of care.

•Refuse care or treatment after the consequences of refusing care or treatment are fully presented.

•Be informed of patient rights to formulate an Advanced Directive, if applicable.

•Have one’s property and person treated with respect, consideration and recognition of patient dignity and individuality.

•Be able to identify visiting personnel members through proper identification.

•Be free from mistreatment, neglect, or verbal, mental, sexual, and physical abuse including injuries of unknown source and misappropriation of patient property.

•Voice grievances/complaints regarding the treatment or care/service, lack of respect of property or recommend changes in policy, personnel, or care/service without restraint, interference, coercion, discrimination, or reprisal.

•Have grievances/complaints regarding treatment or care/service that is (or fails to be) furnished, or lack of respect of property investigated.

•Confidentiality and privacy of all information contained in the patient records and of Protected Health Information.

•Be advised on the homecare policies and procedures regarding the disclosure of patient records.

•Choose a healthcare provider, including choosing and attending physician if applicable.

•Received appropriate care/service without discrimination in accordance with physician’s orders if applicable.

•Be informed of any financial benefits when referred to a homecare facility.

•Be fully informed of one’s responsibilities.
